In signing this application, we hereby waive and release all other contestants, sponsors, host club and tournament officials from all claims for injury and/or damage incurred in connection with this tournament. Each competitor agrees to submit, by his/her signature on this document, to a truth-verification test. Entry constitutes permission to use winners’ names and likenesses for advertising and publicity purposes. Receipt date of entry determines starting position. Cash, check, or money order must accompany this form to qualify for tournament entry. No postdated checks will be accepted.
